Dear Parents (WPS & Turtles)

You are requested to duly fill in the given form if you are willing to avail the school transport facility for your ward for the session 2022-2023. This form will remain open for submission till 4:00 pm on March 13, 2022. Please make sure to fill the form in capital letters.

To avoid any inconvenience regarding availability of transportation, It is mandatory for the parents to fill the form by the given date.

Please note the form is to be filled by REGISTERED PARENTS ONLY of session 2021-2022.
